Sample audit output

Sample Compliance Matrix

A training matrix showing which team members have acknowledged which document versions, with clear status indicators. The single most useful view for understanding your team's training compliance at a glance.

01

What this output is

A compliance matrix (also called a training matrix) is a cross-reference grid showing every team member against every assigned document version. Each cell shows the acknowledgment status: Acknowledged with the completion timestamp, Pending if still within the due date, Overdue if past due, or Not Assigned if the document was not assigned to that person. It is the single most requested document in training audits.

02

When your team uses it

Generate a compliance matrix before any internal or external audit to demonstrate training status across your organization. Use it during management reviews to identify gaps, before product releases to confirm all required training is complete, or on a regular cadence (monthly or quarterly) to track progress. Auditors often ask for this view first because it immediately shows whether training obligations are being met.

Key fields and sections

  • Team member names (rows) with email addresses
  • Document titles and version numbers (columns)
  • Cell status: Acknowledged (with UTC timestamp), Pending, Overdue, or Not Assigned
  • Summary statistics: total acknowledged, total pending, total overdue
  • Organization name and matrix generation date
04

What it proves to an auditor

The compliance matrix proves the overall training posture of your organization at a point in time. It demonstrates that training assignments exist, that completions are tracked by revision, and that overdue items are visible. For ISO 13485 and FDA QMSR audits, it provides the training status evidence that auditors expect to see as part of your quality management system.

05

How ApprovaDoc generates it

In ApprovaDoc, navigate to the Reports section and select Compliance Matrix. The system cross-references all active assignments with the immutable acknowledgment records to build the grid. You can filter by team, document, or status before exporting as PDF or CSV. The data is always derived from write-once records, so the matrix reflects the original, unaltered acknowledgment history.

Common questions

Sample Compliance Matrix — common questions

A compliance matrix is a grid that shows every team member (rows) against every assigned document version (columns), with each cell indicating the acknowledgment status: Acknowledged with timestamp, Pending, Overdue, or Not Assigned. It gives you a single-page view of your entire team's training status.
The PDF is formatted for reading, printing, and sharing with auditors. The CSV contains the same data in a tabular format that can be opened in Excel, Google Sheets, or imported into other tools like a QMS or data analysis software.
The matrix reflects the status at the time it is generated. To get an updated view, generate a new export. The live training dashboard in ApprovaDoc shows real-time status without needing to re-export.
Yes. Before exporting, you can filter by team, document, or status to generate a focused matrix. This is useful when preparing evidence for a specific audit scope or when reviewing training completion for a particular department.
